Kindergarten Registration

Students qualify to attend Kindergarten if they reach the age of 5 before July 31st. If you have a child who meets this age requirement and have to reached out to the school please do so at your earliest convenience. Registration paperwork was sent out to all known children within the Doniphan-Trumbull school district. If you did not receive this paperwork please notify the school and it will be provided to you.

Doniphan-Trumbull registers students according to the Nebraska State Laws. You will need to meet each regulation before your child will be considered to be registered. 

1. Birth Certificate- 

  • You must bring a certified birth certificate from the state. The hospital copy is NOT valid. 

  • If you do not already have a certified copy, you may order the state birth certificate by downloading  Birth Certificate Application.pdf and mailing or can go online to the state's vital records. (dept of H.H.S.) This wll take approximately 2 weeks to obtain by mail. If you are in Lincoln and go to their office, it takes about 20 minutes.

4. Student health physical & eye exam- physicals and eye exams must be complete within 6months of school starting. Any exams outside of 6 months are not valid for registration purposes. The Physical Examination Report will need to be completed by your family physician before the start of school in August. You can download that form here Physical Examination Report.pdf.

5. A copy of your child's immunization records- Your child is not permitted to start school if all immunizations are not in. Student's will be required to be immunized against measles, mumps, rubella, poliomyelitis, diphtheria, pertussis, tetanus, hepatitis, and chicken pox. Exemptions shall be granted for: medical exceptions for health reasons substantiated by a signed statement from a physician and religious conflict substantiated by a signed affidavit.
